Please provide Ms. MH with the following:

• Complete answers to the above document.
• Stamped, addressed envelopes (one for each institution)
• The school’s recommendation form (be sure to fill it out and sign it).
• Put everything together in one large envelope and write your name on the front.

Please note that it is Ms. MH’s policy not to share the recommendation with you.  You must waive your right to see the recommendation if you would like her to write it.  Be assured that if Ms. MH cannot write you a good recommendation, she will let you know when you make your request.
